Building Digital Health - The Coding Cycle of Chronic Disease

Identifying patients with a possible chronic disease can ensure they are provided with the right care at the right time. The CAT Plus software suite can help you identify patients who may have fallen though the gaps and are missing from your current chronic disease registers. This event will provide you with the opportunity to see the CAT Plus software suite in action. You will also have the opportunity to discuss strategies to implement data completeness and with your peers.

This session will provide you with knowledge on:

• How to easily identify patients who could be at risk of developing chronic disease
• How to build chronic disease registers
• Identifying revenue potential in the area of chronic disease management
• Discuss with your peers how to implement change in your practice
